Mode of action
Inhibits bacterial cell wall synthesis by binding to one or more of the penicillin binding proteins (PBP’s); Which in turn inhibits the final transpeptidation step of peptidoglycan synthesis in bacterial cell walls, thus inhibiting cell wall biosynthesis. Bacteria eventually lyse due to ongoing activity of cell wall autolytic enzymes (autolysins and murein hydrolases) while cell wall assembly is arrested.

Contraindications: Contraindicated in patients with history of previous hypersensitivity reaction to any of penicillin or any component of the formulation.

Precautions: Pseudo-membranous colitis has been reported with nearly all-antibacterial agents, and may range from mild to life threaten. Therefore, it is important to consider this diagnosis in patients who present with diarrhea subsequent to the administration of antimicrobial agents. Super infections with fungal or bacterial pathogens should be kept in mind during therapy.

Dosage:
Capsules: Adults: 250-500 mg 6 to 8 hourly or more frequently according to severity of the infections.

Syrup: Adults: 250-500mg 6 to 8 hourly or more frequently according to severity of the infections.
Children: 2 to 10 years : 125 to250 mg 6 to 8 hourly
Children: Upto 2 years : 75 to 150 mg 6 to 8 hourly
Administer: ½ to 1 hour before meals or as directed by the registered physician

Paediatric Drop: Paediatric drops are recommended for children under 6 month of age based on body weight 35- 100 mg/kg/day ½ to 1 hour prior to feeding or as directed by the registered physician.

Special dosage recommendations: Typhoid and para typhoid
Adults: 4 g daily in divided doses for 14 to 21 days.
Children: 100 mg/kg/day in divided doses for 14 to 21 days.
Typhoid Carrier State 3 to 4 g daily in divided doses for minimum of one month.

Gonorrhoea: A single dose of 3 g plus 1 g probenecid where infection is caused by penicillin sensitive organisms

Meningitis: Adults: 150 mg/kg/day in equally divided doses.

Syphilis: A dosage of 250 mg every 6 hours has been used in all stages of Syphilis. Duration of treatment varied form 4 weeks upto 5 months according to the stage of disease and serological response.

Ottitis Media: In severe or recurrent ottitis media, especially where compliance may be a problem 750 mg twice a day for 2 days may be used in children of 3 to 10 years.
